Home & Community Palliative Care will also consider how healthcare professionals can coordinate services between hospitals, hospices, primary care, and community organizations. Research and practical experiences will provide insights into improving accessibility and patient satisfaction. The track aims to strengthen community-based palliative care and provide patients with compassionate, coordinated, and personalized support closer to home.
